Our Unique Approach

Our company’s unique approach addresses the total well-being of our clients and their families through a comprehensive program that includes assessment, planning, service coordination, advocacy, and direct care by our well-trained care team, which is led by a “Quality Care Manager.” Without the type of care offered by Passion Home Health Care (PHHC), people with chronic conditions would be placed in nursing homes, dementia units, or other types of long-term care institutions. Our Care Team

Passion Home Health Care direct care workers are defined by a set of important criteria. They are employees and caregivers integrated under a common code of conduct and training to treat the person they help as a beloved individual, a cherished companion, and a person who deserves to be treated with dignity and respect.

Our Care Managers

An expert quality care manager is assigned to supervise and care to coordinate the delivery of all aspects of client care. The quality care or intake manager can be a case manager or scheduling coordinator, depending on the nature of the client’s needs. This is the point person coordinating all the care received by each client.

Care Plan Oversight

The quality care manager oversees the care plan for each client, including both long- and short-term care, based on an integrated home assessment made at the client's home. They schedule the direct care worker or caregiver, resolve scheduling conflicts, make in-home functional assessments, and coordinate additional services needed, such as transportation, social stimulation, counseling, and in-service training for the maintenance of a safe environment.

Family & Provider Coordination

Questions from family or physicians about client care are addressed by the quality care manager. The quality care manager's additional responsibilities also include providing advice and coordination and collaboration with professionals and outside providers. They also identify changes in the client's condition and recommend when a new team member with a separate specialty needs to be involved in the case.

Understanding the Individual

Each consumer is different in terms of personal care; therefore, every person needs to be understood within the context of their environment and personal history. To arrive at an individual's level of care, a care plan must address their individual need in a way that is uniquely their own.

Our Passion Home Health Care licensed professionals will conduct an in-depth assessment of each individual’s needs, preferences, strengths, support systems, and resources in consultation with all concerned parties.

Careful Planning, Thoughtful Care

Our professionals listen and learn before they respond with solutions and services. The input we receive enables us to understand all of the clinical and psychosocial reasons that will influence the customization of the care plan.

Our Home Health Care Aides and Companions

All of our home health aides and companions are chosen based on the most rigorous and selective process in the industry. The goal of our screening process is to select only dedicated and outstanding people who hold caring as one of their most important personal values.

Dedicated Care Professionals

We recognize that they are the foundation of the care we provide our clients. Our home health aides all have certificates in home health care. Their responsibilities are to assist the client with activities of daily living, such as eating, bathing, dressing, providing medication reminders, and shopping for groceries. They make sure the client is as comfortable, confident, and pain-free as possible.

Passion Home Health Care will also be flexible in working with friends, relatives, and family members of the client only if they obtain PHHC training and are certified.
